Please note: This notification form is ONLY for the Greater Wellington region. 

If you're notifying about an illness outside of the Greater Wellington region, you can find your local Public Health service and contact them directly from the Te Whatu Ora website.

 

This form is to be completed when 15% or more of your centre or kōhanga (including staff and tamariki) have a suspected outbreak. If you have any questions please email HealthProtection@huttvalleydhb.org.nz.

Please note not to put any personal health information into this notification. Other than the name and contact number of the person notifying.

An outbreak is:

  • Three or more cases (staff or tamariki) in one area (e.g. under-twos room) showing similar symptoms of illness in 48 hours OR 15% of the centre are unwell.

  • Please be reminded of the requirement outlined under HS34 of the Licencing criteria for centre-based ECE services: Where there is a serious injury or illness or incident involving a child while at the service that is required to be notified to a specified agency, the service provider must also notify the Ministry of Education at the same time:
